School legislation authorizes the loan of textbooks, instructional materials, and equipment by the Secretary of Education to Pennsylvania children enrolled in Kindergarten through Grade 12 in nonpublic and private schools. Our school is now in the process of requesting specific textbooks, materials, and equipment to be loaned to your child/children. It is required, however, that a parent/guardian of each child attending the nonpublic or private school individually requests the loan of textbooks, instructional materials, and equipment. This program is available only to Pennsylvania residents.
I, the undersigned, understand that if my child/children is admitted to Mother of Divine Grace Interparochial Catholic School and I agree to Continuous Enrollment, I am obliged to pay all tuition, fees, and expenses for that year. I also understand that all application fees are non-refundable. Tuition must be current to maintain continuous enrollment status.

Each enrollment period, parents will be given the opportunity to opt-out of continuous enrollment for the following school year by going to the school's website and filling out the electronic Opt-Out / Withdrawal form.
